If you plan to register in person or by mail, please make sure that all pages of the applicable registration form are completed in full, signed by the appropriate person(s) and returned to the club by mail or at one of the registration dates.
Birth certificates are mandatory for new registrants.

Please ensure that you complete all applicable areas and sign the registration form before mailing. New registrants MUST include a copy of their birth certificate with the form for verification purposes. Copies of birth certificates will be shredded after verification.

NOTE: Scheduled nights for the age groups are NOT guaranteed and are subject to change depending on registration turnout. League age groupings are also subject to change, depending on registration turnout.
U6 (born 2011) preferred game night selection is also not guaranteed. The Club will place players based on availability of space and may move players, as necessary, to balance the league.
Attention Players and Parents:
The Club will not entertain requests for player placements. The Club has to ensure that teams are properly balanced to make sure it is fun for all who participate. Thank you for your cooperation.
A late fee of $50.00 applies to all youth registrations after April 1, 2017, as well as registrations for which payment has not been mailed or received by April 1, 2017.

1. All refund requests shall be made in writing and properly documented.
Refunds or credits may be requested only for serious injury or major medical issues confirmed by a doctor’s certificate. Please remember that a lack of volunteers does not constitute a valid refund request! As it is not possible for the club to know what registration numbers will be in any age group, advertised practice/game nights and league age groupings are NOT GUARANTEED.
2. An administration fee of $50.00 per player applies to refund requests

All requests for refunds should be forwarded to the Club Registrar, in writing, and will be processed at the discretion of the Board of Directors. Any refunds issued will be pro-rated to the latest date that all necessary, relevant documents are received by the Club. No refunds will be made after May 1, 2017 for youth programs except in the event of serious injury or medical issues confirmed by a doctor’s certificate. Refunds will not be paid until the uniform has been returned to the Club. Please remember that a lack of volunteers does not constitute a valid refund request!
